How to get your provisional licence?

Guaranteed Pass' Starter Pack helps you to begin your journey towards your LGV drivers licence.

There are two simple steps to get you started to achieving your provisional LGV licence with Guaranteed Pass.

Obtain your DVLA Medical Form (D4)

Before you apply for your provisional licence, you must obtain the DVLA Medical Form (D4).  To do this, you can either visit your GP or our recommended Driver Medical Specialist.  Click on www.driversmedicals.com for fees and further information.

Obtain your DVLA Application for Large Good Vehicle (LGV) or Passenger Carrying Vehicle (PCV) driving licence (D2)

Once you have otained your medical form, you are ready to apply for your provisional licence.  You must send the completed D2 form with your medical form to the DVLA Swansea address.  If you do not have a photocard style licence already, you would need to change this too.  For contact details click www.dvla.gov.uk/contactus/drivers_enquires.aspx
